48 year old male diagnosed with Philadelphia chromosome positive B-acute lymphoblastic leukaemia treated to complete remission with chemotherapy and imatinib. Subsequently presented with acute bilateral lower limb weakness. Demonstrated here is the CSF with a marked number of lymphoblasts and numerous mitotic figures. FISH studies on CSF confirmed BCR-ABL1 gene fusion. Bone marrow aspirate and trephine was normal, confirming isolated CNS relapse of ALL.
