- As the corresponding author, who has obtained the assent of all the other authors, I hereby confirm the assignment of all copyrights in and to the atlas image set named in this submission in all forms and media, now or hereafter known, to the American Society of Hematology ("ASH") effective if and when the image set is accepted for publication as part of the ASH Image Bank.
- I confirm that publication of the submission, including all text, images, photographs, illustrations, tables, and diagrams accompanying or forming part of the submission, by ASH will not violate any copyright or other intellectual property right or any right of privacy or publicity of any person or entity, is not otherwise contrary to law, and that the submission and all accompanying material are not libelous.
- I confirm that any potential conflicts of interest were disclosed at the time the image set was submitted.
- I will obtain written permission from any respective copyright owners for the use of any text, images, photographs, tables, diagrams, or other material accompanying the submission that have been previously published or that others have a copyright or other proprietary interest in. I agree that it is my responsibility to pay any fees charged for such permissions. Do you have rights/permission to reproduce this image set?

I represent and warrant that any images that I have uploaded as part of the submission process do not contain any patient identifiable material or, if any images contain any patient identifiable material, I have obtained informed patient consent in a document. If informed patient consent has been obtained, I represent and warrant that I have archived this document in my possession and will contact ASH so that notice may be published.
