33-year-old male presented with splenomegaly. Giemsa stained peripheral blood smear showed marked leukocytosis with immature myeloid precursors, mature neutrophils and 8% basophils. Erythroid precursors were also seen (87 per 100 WBCs). Erythroid precursors showed marked dysplastic features. The patient was positive for Philadelphia chromosome and BCR-ABL1 fusion transcript.
