Patient with recurrent adenopathy with concern for lymphoma. Increased intrafollicular IgG4+ plasma cells are present. This case otherwise showed follicular hyperplasia with polyclonal plasma cells.

Such intrafollicular plasma cells are often seen in cases of IgG4-related lymphadenopathy. The current criteria requires >100 IgG4+ plasma cells/HPF and >40% IgG4+ fraction within all IgG+ plasma cells for establishing the diagnosis of IgG4-related lymphadenopathy.
