A small field of red pulp at a higher magnification is illustrating cytoplasmic and nuclear features. There are occasional plasma cells; however, the main population composed of mixed population of lymphoid cells. Occasional larger transformed appearing cells may be a concern for a neoplastic infiltrate especially if there is no clinic information of possible viral infection. Although some patients with infectious mononucleosis involving lymphoid tissues were known to have large areas of transformed B-cell infiltrates, the case shown here had predominantly normal appearing white pulps.
